The most authentic Japanese udon / tempura so far...
...is surprisingly, in Manhattan.

I wrote a review on yelp here. But seriously if you're looking for a good spot in Manhattan near Grand Central, I can't recommend this (and the black sesame ice cream) highly enough.

I was also recently very taken with a Japaneses Restaurant called Akira, on (I think it was) 1st Ave between E 91st and E 92nd Streets. Addicitive hot green tea, insanely fresh non-reconsituted white miso soup, excellent bento box lunches and super-fresh sushi. I think I'm in love...

Yea, for some reason, I'm told that the most authentic Japanese places are all around the upper east side of Manhattan.

If you go for an udon tempura combo, make sure you ask for the tempura to be placed on the side instead of on top of the noodles. I always find that the udon comes out steaming hot, and by the time it's cool enough to eat, the tempura is too soggy. Of course, that doesn't apply if you like soup-soaked tempura.
